Napoli have had little option for squad rotation considering the massive injury crisis and some of the fatigue showed in a midweek 2-2 draw with relegation-threatened Hellas Verona.
The Partenopei had been trailing 2-0 before the comeback, with goals from McTominay and Giovanni Di Lorenzo.
It was hoped that David Neres might be available after an ankle sprain, but he has not made the trip, with long-term absentees Andre Frank Zambo Anguissa, Billy Gilmour, Kevin De Bruyne and Romelu Lukaku still out of action.
Leonardo Spinazzola and Juan Jesus return to the starting XI, with Politano, Hojlund and versatile Eljif Elmas upfront.
Napoli have not won a Serie A match away to Inter since April 2017, but the last two editions both ended 1-1.
